As a Hospice Admission Registered Nurse / RN, you will enable patients to spend quality time with their loved ones and doing the activities they enjoy. You will assess patients for hospice eligibility and explain hospice care services to patients, families/caregivers. You will ensure all patient’s initial needs are addressed and collaborate with the Inter-Disciplinary Team members regarding the initial plan of care, patient’s condition, and other essential information. Requirements

RN license in the state you work
Two years of experience as a registered nurse, hospice experience preferred
Current driver’s license
Current CPR and BCLS certification

Responsibilities
