About The Position

Provides direct and indirect nursing care. The nurse is accountable for the delivery of patient care utilizing the nursing process within the standards, policies, procedures and guidelines of the organization. Makes skilled nursing assessments of clients/caregivers in the homecare setting which includes physical, psychosocial, and environmental components. Develops, revises, and updates care plans in consultation with primary physician, client, and caregiver. Coordinates all healthcare team members including client/caregiver to attempt to achieve the goals of the Care Plan.

Requirements

  • Associate’s degree in nursing
  • Must have MN or WI Registered Nurse (RN) license upon start date.
  • Must provide proof of MN and WI Registered Nurse (RN) license within 4 months of start, if both MN and WI Registered Nurse (RN) licenses are not present on start date.
  • Current BLS – American Heart Association or BLS – American Red Cross (CPR/AED for Professional Rescuer & Health Care Provider)
  • Must have a dependable vehicle, valid driver’s license, auto insurance and meet all criteria required under Lakeview’s Automobile Liability Guidelines
  • Knowledge of reimbursement procedures as well as knowledge of community resources
  • Able to function independently with minimal supervision in home setting
  • Strong assessment skills
  • Ability to read, write and speak the English language
  • Ability to carry out directions
  • Computer experience

Nice To Haves

  • Bachelor’s degree in Nursing
  • Proof of application for alternate state within 4 weeks of start, if both MN and WI Registered Nurse (RN) licenses are not present on start date.
  • Previous related healthcare experience
  • At least 1 year of experience in Acute Care, Long Term Care or Short-Term Care experience or equivalent
