About The Position

Under the supervision of the Clinical Manager, the RN field clinician is accountable for assessing patients for eligibility for homecare services, admitting eligible patients following the admission process, and managing patients from admission through transition into the community. The Baylor RN is required to work two (2) 11.5 shifts Friday and Saturday, Saturday and Sunday, or Sunday and Monday. They work 23 hours and are paid for 34.5 hours.

Requirements

  • Equivalent education required for RI Registered Nurse licensure.
  • A minimum of one (1) year progressive clinical experience.
  • Valid RI Registered Nurse license.
  • Valid CPR certification.
  • Valid driver’s license and automobile liability insurance.

Nice To Haves

  • Bachelor’s Degree in relevant discipline strongly preferred.
  • Previous experience in an acute care or Home health care environment setting preferred.

Responsibilities

  • Admissions, revisits, resumptions of care, recertifications, and discharges to ensure safe and effective patient care.
  • Coordinates interdisciplinary care.
  • Monitors and adjusts treatment to achieve positive therapeutic outcomes.
  • Identifies opportunities for process improvement.
  • Carries out responsibilities in accordance with organization policies and procedures, as well as Federal, State, and accreditation regulations.
