About The Position

This role is in-person, community based, covering Northumberland County. As the Nurse Home Visitor you'll act as a case manager and implement standard protocols for families enrolled in the Nurse Family Partnership. A typical schedule is part-time, 20 hours weekly, 8 am to 4:30 pm with the flexibility to work occasional evenings to accommodate client needs. This role covers Northumberland County; you’ll complete home visits each workday. In this community‑based position you’ll meet clients in their homes and other in‑person community settings.

Requirements

• Licensed Registered Nurse (Pennsylvania)
• Basic Life Support Certification
• At least 2 years of RN OB and/or Pediatric experience strongly preferred.
• Previous nursing experience in home visits, behavioral health, or public health is helpful.
• Minimum of 2 years-Clinical experience

Responsibilities

• Assess physical, emotional, social, and environmental needs of women and their families.
• Provide guidance and instructions to mothers and significant others regarding pregnancy, infants, and toddlers based on the standardized approach of the Nurse Family Partnership.
• Provide overall coordination of care and home visitation for families enrolled in the Nurse Family Partnership.
• Implement Telehealth per National service office guidelines and agency policy.
• Assess physical, emotional, social and environmental needs of women and their families as they relate to the Nurse Family Partnership domains.
• Initiate a plan of visitation based on the standardized protocols of the Nurse Family Partnership.
• Assist women and their families in establishing goals and outcomes.
• Provide anticipatory guidance and instruction to mothers and significant others regarding pregnancy, infants and toddlers based on the standardized protocols of the Nurse Family Partnership.
• Provide education, support, and referral resources in assisting women and their families in attaining their targeted goals.
• Evaluate the effectiveness of the plan of visitation and the outcome of each visit.
• Complete and submit all appropriate documentation forms within 48 hours.
• Provide for discharge planning.
• Educate referral sources and the public about the Nurse Family Partnership.
• Develop and maintain community relationships to support client referrals.
• Complete all required Nurse Family Partnership education and orientation.
• Develop therapeutic relationships, utilize concepts of reflection and motivational interviewing with women and their families in a home visiting environment.
• Participate in review and analysis of reports and other reports available through Nurse Family Partnership for achievements and areas for improvement.
