About The Position

This role involves providing direct patient care in a home health setting. Key responsibilities include triage, blood draws, wound care, and executing physician orders while meticulously monitoring and documenting patient conditions. The nurse will also develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team (IDT). Coordination of care across different disciplines and support for discharge planning are crucial to ensure continuity of care. Additionally, the role involves educating patients, caregivers, and staff, and participating in team and care plan meetings. The position requires participation in an on-call rotation and travel to patient residences or care facilities. This is a part-time position (20 hours per week) with a flexible schedule and documentation options.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required.
Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

2 years of hospital or home health experience strongly preferred.
