About The Position
Navitas Healthcare, LLC is seeking a Home Health Registered Nurse (RN) for an exciting Travel or Local job in St. Johnsbury, VT. This is a contract position for 13 weeks, with a shift and schedule of Days, Monday-Friday, and a possible weekend rotation, guaranteeing 36 hours per week.
Requirements
- Active Vermont RN License required
- Minimum 1 year of clinical nursing experience preferred
- Current BLS certification required
Nice To Haves
- Home Health experience preferred
- Experience with Homecare Homebase (HCHB) is a plus
Responsibilities
- Perform patient admissions, evaluations, and care plan development
- Deliver skilled nursing care in a home health setting
- Collaborate with interdisciplinary healthcare teams and paraprofessionals
- Support coordinated, patient-focused care within the community
- Maintain accurate documentation using Homecare Homebase (HCHB)
