About The Position

Sage Group HealthCare Staffing is seeking Home Health Registered Nurses for 13-week travel/contract positions in Everett, WA. This role involves providing and coordinating care for patients in their homes, developing and implementing nursing care plans, supporting patient recovery, and educating patients and families on disease management. The position also includes case management, clinical oversight, and ensuring adherence to nursing practice standards and documentation requirements. The clinician will cover Everett, WA, and surrounding areas within a 50-mile radius.

Requirements

• Registered Nurse: 2 years experience required.
• Recent/Current Home Health Nursing experience: 1 year within the last 3 years required.
• OASIS experience required.
• Home Care Home Base (HCHB) charting required.
• Clinician will be expected to complete the documentation by 10 am the following day.
• Must have valid Driver’s License and Personal Auto Insurance.
• ASN or an equivalent required.
• RN License for the state of Washington or Compact if applicable required.
• BLS Certification from AHA or American Red Cross required.

Nice To Haves

• BSN or an equivalent highly preferred.

Responsibilities

• Provide and coordinate care for assigned patients in the comfort of their homes.
• Develop and implement nursing care plans for all types of visits, including Start of Care and admissions.
• Support patient recovery and their ability to stay at home.
• Empower patients and families to reach healthcare goals by educating them on disease management.
• Perform case management and provide clinical oversight to the interdisciplinary team.
• Provide nursing care and services within the standards and scope of nursing practice as defined by state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
• Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
• Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
• Notify physicians of significant changes in patient status.
• Document and complete charting for all patient visits within 24 hours.

• Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments.
