About The Position

Sage Group HealthCare Staffing is seeking Home Health Registered Nurses for 13-week travel/contract positions in Walla Walla, WA. Both local and traveler candidates are accepted. The role involves providing and coordinating patient care in their homes, developing and implementing nursing care plans for various visits including Start of Care and admissions. The goal is to support patient recovery, enable them to stay at home, and empower patients and families through education on disease management. Responsibilities include case management, providing clinical oversight to an interdisciplinary team, and delivering nursing care within professional standards and facility policies. This includes ensuring physician orders are transcribed correctly, assessing patient status, documenting changes, and completing charting within 24 hours. The territories to cover include Walla Walla County and parts of Columbia and Umatilla Counties.

Requirements

  • Registered Nurse: 2 years - Required
  • Recent/Current Home Health Nursing experience: 1 year within the last 3 years - Required
  • OASIS experience - Required
  • Home Care Home Base (HCHB) charting - Required
  • Must have valid Driver’s License and Personal Auto Insurance
  • ASN or an equivalent – Required
  • RN License for State of the state applying or Compact if applicable - Required
  • BLS Certification from AHA or American Red Cross - Required

Nice To Haves

  • BSN or an equivalent – Highly preferred

Responsibilities

  • Provide and coordinate care for assigned patients in comfort of their homes.
  • Develop and implement nursing care plans for all types of visits including Start of Care and admissions as assigned.
  • Support patient recovery and ability to stay at home.
  • Empower patients and families to reach healthcare goals by educating them on disease management.
  • Case management and provide clinical oversight to interdisciplinary team.
  • Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
  • Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
  • Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
  • Notify physicians of significant changes in status.
  • Document and complete charting for all patient visits within 24 hours.
