In-Home Health - Physician (Part Time) - Wenatchee, WA
About The Position
Signify Health, part of CVS Health®, is seeking a Part-time Physician to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care and build trusted relationships. The physician will conduct comprehensive health evaluations, including medication and medical history reviews, physical assessments, and potentially diagnostic tests. The goal is to provide flexible, comprehensive, and personalized health evaluations directly to patients. The physician will use an iPad with an intuitive clinical workflow and is not required to prescribe medication, order lab tests, or alter current treatment plans. The primary workplace is the patient's private residence, which can vary in conditions. Candidates must be comfortable working in diverse home environments, potentially including pets, secondhand smoke, and airborne allergens.

Responsibilities
- Conduct In-Home Health Evaluations, lasting approximately 45 minutes.
- Perform medication and medical history reviews.
- Conduct physical evaluations.
- Perform additional diagnostic tests if ordered by the health plan (e.g., diabetic eye exams, spirometry).
- Use an iPad and an intuitive clinical workflow for each evaluation.

- Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
- Be flexible to travel locally and within licensed states, as needed.
- Conduct virtual visits as needed.
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.).
