About The Position

Holland Hospital is seeking a compassionate and independent Home Health Registered Nurse (RN) to deliver high-quality, patient-centered care in patients' homes. The role involves performing comprehensive assessments, developing and implementing individualized care plans, providing skilled nursing interventions, and collaborating with physicians and interdisciplinary teams to promote patient safety, independence, and optimal outcomes. This position emphasizes strong clinical judgment, effective communication, and patient/family education. The role utilizes ambient AI documentation technology to reduce administrative burden while maintaining RN oversight and regulatory compliance. This position offers more time with patients, autonomy with support from a collaborative team, opportunities to build meaningful connections, and modern nursing practice with innovative tools and flexible scheduling.

Requirements

  • Graduate of accredited Registered Nursing Program required
  • Current Michigan RN License (or completed by end of orientation) required
  • Current BLS for the Healthcare Provider (or completed within 60 days of hire) required

Nice To Haves

  • Recent hospital experience and/or previous nursing practice experience preferred.
  • 2 years prior RN experience in desired specialty area preferred.
  • Prior experience with electronic patient documentation system preferred.
  • Prior experience with technology common to specialty area (i.e., cardiac monitors, IV pumps) preferred.
  • Bilingual preferred.

Responsibilities

  • Performs comprehensive assessments.
  • Develops and implements individualized plans of care.
  • Provides skilled nursing interventions.
  • Partners closely with physicians and interdisciplinary team members.
  • Promotes safety, independence, and optimal outcomes.
  • Conducts holistic assessments.
  • Manages individualized plans of care.
  • Coordinates services across the interdisciplinary team.
  • Monitors patient progress throughout the episode of care.
  • Serves as the primary point of clinical oversight.
  • Ensures care is delivered safely, efficiently, and in alignment with physician orders, regulatory requirements, and patient goals.
  • Supports successful outcomes and transitions across the continuum of care.
  • Collects and documents comprehensive data pertinent to the patient's health or the situation.
  • Uses critical thinking skills to analyze assessment data to determine the pertinent diagnosis or issues.
  • Responds appropriately to changing needs of patients, including response to emergency/life threatening situations.
  • Identifies expected outcomes for a plan individualized to the patient or situation.
  • Develops and documents an individualized plan of care which incorporates the patient's significant other as appropriate and that prescribes strategies and alternatives to attain expected outcomes.
  • Identifies initial and ongoing discharge planning needs.
  • Evaluates and documents the patient's response to care given.
  • Coordinates care across the continuum to meet the identified and anticipated needs of the patient and his/her support network.
  • Functions as advocate for patient/significant other.
  • Demonstrates skills needed to implement the identified plan of care.
  • Prioritizes and adjusts priorities based on patient/unit needs.
  • Evaluates and documents patient's progress towards attainment of desired outcomes.
  • Evaluates a patient's learning abilities and carries out the education of the patient.
