About The Position

Holland Hospital is seeking a compassionate and independent Home Health Registered Nurse (RN) to deliver high-quality, patient-centered care in patients' homes. The role involves performing comprehensive assessments, developing and implementing individualized care plans, providing skilled nursing interventions, and collaborating with physicians and interdisciplinary teams to promote patient safety, independence, and optimal outcomes. The position emphasizes strong clinical judgment, effective communication, and patient/family education. The use of ambient AI documentation technology is incorporated to reduce administrative burden and allow more focus on patient care. The Home Health RN acts as both a clinician and case manager, ensuring care is safe, efficient, and aligned with physician orders, regulatory requirements, and patient goals.

Requirements

  • Graduate of accredited Registered Nursing Program required
  • Current Michigan RN License (or completed by end of orientation) required
  • Current BLS for the Healthcare Provider (or completed within 60 days of hire) required

Nice To Haves

  • Recent hospital experience and/or previous nursing practice experience preferred.
  • 2 years prior RN experience in desired specialty area preferred.
  • Prior experience with electronic patient documentation system preferred.
  • Prior experience with technology common to specialty area (i.e., cardiac monitors, IV pumps) preferred.
  • Bilingual preferred.

Responsibilities

  • Perform comprehensive patient assessments.
  • Develop and implement individualized plans of care.
  • Provide skilled nursing interventions.
  • Partner with physicians and interdisciplinary team members.
  • Promote patient safety, independence, and optimal outcomes.
  • Conduct holistic assessments.
  • Manage individualized plans of care.
  • Coordinate services across the interdisciplinary team.
  • Monitor patient progress throughout the episode of care.
  • Ensure care is delivered safely, efficiently, and in alignment with physician orders, regulatory requirements, and patient goals.
  • Support successful outcomes and transitions across the continuum of care.
  • Collect and document comprehensive data pertinent to the patient's health.
  • Use critical thinking skills to analyze assessment data.
  • Respond appropriately to changing patient needs, including emergencies.
  • Identify expected outcomes for a plan individualized to the patient.
  • Develop and document an individualized plan of care.
  • Identify initial and ongoing discharge planning needs.
  • Evaluate and document patient's response to care.
  • Coordinate care across the continuum.
  • Function as an advocate for patient/significant other.
  • Implement the identified plan of care.
  • Prioritize and adjust priorities based on patient/unit needs.
  • Evaluate and document patient's progress towards desired outcomes.
  • Optimize patient learning through evaluating learning abilities and carrying out patient education.
  • Maintain the policies and procedures of the hospital and nursing department.
  • Practice in accordance with the ANA Standards of Professional Performance.
  • Direct and supervise support personnel in the performance of delegated nursing activities.
  • Participate in or support shared decision-making activities.
  • Demonstrate awareness of and accountability to department/organization strategic goals and quality initiatives.
