Phoenix Home Care and Hospice is seeking a Home Health RN Case Manager in Maryland Heights, MO. This full-time, Monday-Friday position offers a unique opportunity to manage a caseload of home health patients with real autonomy and support. Requirements
- Missouri or Compact RN license
- Valid driver's license
- Strong organizational skills

Responsibilities
- Manage a caseload of home health patients.
- Supervise LPN visits.
- Coordinate with Physical Therapy, Occupational Therapy, and Speech Therapy teams.
- Conduct assessments.
- Build personalized care plans aligned with physician orders.
- Support patients managing conditions like CHF, COPD, diabetes, wounds, and medication regimens.
