Requirements
- Missouri or Compact RN license
- Valid driver's license
- Strong organizational skills
- Strong clinical skills
- Strong communication skills
Responsibilities
- Manage a caseload of home health patients.
- Supervise LPN visits.
- Coordinate with Physical Therapy, Occupational Therapy, and Speech Therapy teams.
- Conduct patient assessments.
- Build personalized care plans aligned with physician orders.
- Support patients managing conditions such as CHF, COPD, diabetes, wounds, and medication regimens.

- $78,000–$88,000+ base salary (pay-per-visit model with potential to out-earn the range)
