About The Position

This position is for a Registered Nurse in Home Health Case Management, serving the west side of Chicago, including Logan Square and surrounding communities. The role involves developing and maintaining therapeutic relationships with patients and families, ensuring quality and cost-effective care, providing patient and staff education, utilizing the nursing process, and demonstrating leadership and collaboration. The position requires a valid RN license, a driver's license, and one year of medical-surgical experience. The schedule is Monday through Friday, with rotating weekend and on-call responsibilities, including weeknights and every third weekend. A sign-on bonus of up to $10,000 is available for eligible candidates.

Requirements

  • Nurse, Registered (RN)
  • A valid driver’s license issued by the Division of Motor Vehicles.
  • Graduate from an accredited school of nursing
  • One year of medical surgical experience
  • Has as demonstrated proficiency for the requirements for RN role
  • Good communication and interpersonal skills
  • Good documentation skills.
  • Computer skills
  • Good time management and organizational skills
  • Familiarity with patient care equipment
  • Current Illinois RN license
  • Current CPR
  • Ability to work weekends, holidays, and different shifts in order to accommodate staffing
  • Ability to take “on call” assignments.
  • Ability to handle multiple tasks and complete work within short timeframes.
  • Ability to transport objects, patients, and equipment
  • Ability to respond quickly to patient needs and work at fast pace
  • Routinely works with potentially infectious specimens and patients
  • Ability to respond to patients in a timely manner and access all areas of the unit
  • Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
  • Must be able to travel to all areas within the Advocate service area in all extremes of weather on all types of roads.
  • Must be able to assist with ambulation including turning and transferring patients up to 200 pounds
