Home Health Case Manager - Registered Nurse (RN)
About The Position
York General is seeking a compassionate, organized, and self-motivated Registered Nurse Case Manager to join our Home Health team. In this role, you will coordinate and manage patient care in the home setting, helping individuals achieve their health goals while maintaining independence and quality of life. As a trusted healthcare partner, you will collaborate with patients, families, physicians, and interdisciplinary team members to develop individualized care plans, provide skilled nursing services, and ensure seamless care across the continuum. This is an ideal role for an RN who enjoys building relationships, working independently, and making a direct impact on patient outcomes.
Requirements
- Current Nebraska Registered Nurse (RN) license.
- Current BLS certification.
- Valid Nebraska driver's license.
- Basic computer proficiency, including electronic medical record documentation and patient care coordination.
- Self-motivated, compassionate, organized, and detail-oriented.
- Comfortable with working independently in the field.
Responsibilities
- Coordinate and manage patient care plans to ensure high-quality, patient-centered outcomes.
- Conduct comprehensive nursing assessments in the home setting.
- Collaborate with patients, families, physicians, and other healthcare professionals to develop and implement individualized care plans.
- Monitor patient progress and modify care plans as needed.
- Provide skilled nursing services as ordered by the physician.
- Administer medications, treatments, immunizations, and other prescribed interventions.
- Educate patients and caregivers regarding disease management, medications, treatments, and available resources.
- Advocate for patients and help navigate healthcare services.
- Maintain accurate and timely electronic medical record documentation.
- Supervise and collaborate with home health aides and other members of the care team.
- Ensure patient needs are continually assessed and addressed to promote optimal outcomes.
