About The Position
This is a full-time Home Health Registered Nurse Case Manager position serving the South East Chicago area, including neighborhoods like Auburn, Gresham, Brainerd, Princeton Park, Beverly View, Washington Heights, Roseland, Cottage Grove Heights, Pullman, East Beverly, Beverly, and Morgan Park. The role involves developing and maintaining therapeutic relationships, providing individualized care, and ensuring quality, cost-effective patient care. The position requires assessing patient and family needs, implementing care plans, and collaborating with healthcare professionals. It also involves patient and staff education, utilizing the nursing process for problem-solving, and demonstrating leadership and professional development. The role requires a valid driver's license and the ability to travel within the Advocate service area in various weather conditions. Requirements
- Graduate from an accredited school of nursing
- One year of medical surgical experience
- Current Illinois RN license
- Current CPR
- A valid driver’s license issued by the Division of Motor Vehicles.
- Good communication and interpersonal skills
- Good documentation skills.
- Computer skills
- Good time management and organizational skills
- Familiarity with patient care equipment
