About The Position

This is a full-time Home Health Registered Nurse Case Manager position for South East Chicago. The normal office hours are Monday through Friday 8:00 am to 5:00 pm. The position includes weekend rotation availability every 4th weekend and 2 holidays per year for coverage. It also includes rotating after-hours on-call coverage for PRN patient concerns, equitably distributed across the area nursing team. A sign-on bonus of up to $10,000 is available for eligible candidates. This role involves developing and maintaining therapeutic relationships, providing individualized care, assessing patient and family readiness, and mobilizing appropriate resources. The nurse will adapt practice to the latest standards, participate in quality improvement initiatives, and meet regulatory requirements. Education of patients and families, as well as peers, is a key responsibility. The role requires demonstrating the nursing process, making sound clinical decisions, prioritizing care, initiating referrals, and coordinating discharge planning. Leadership, collaboration, and professional development are also essential components, including maintaining current knowledge, effective communication, and critiquing one's own practice.

Requirements

• Nurse, Registered (RN)
• A valid driver’s license issued by the Division of Motor Vehicles.
• Graduate from an accredited school of nursing
• Earn 10 continuing education credits annually
• One year of medical surgical experience
• Has as demonstrated proficiency for the requirements for RN role
• Good communication and interpersonal skills
• Good documentation skills.
• Computer skills
• Good time management and organizational skills
• Familiarity with patient care equipment
• Current Illinois RN license
• Current CPR
• Ability to work weekends, holidays, and different shifts in order to accommodate staffing
• Ability to take “on call” assignments.
• Ability to handle multiple tasks and complete work within short timeframes.
• Ability to transport objects, patients, and equipment
• Ability to respond quickly to patient needs and work at fast pace
• Ability to respond to patients in a timely manner and access all areas of the unit
• Ability to lift up to 100 pounds occasionally and/or in excess of 50 pounds frequently and/or in excess of 20 pounds constantly
• Must be able to travel to all areas within the Advocate service area in all extremes of weather on all types of roads.
• Must be willing to work occasional overtime if needed.
• Must be able to assist with ambulation including turning and transferring patients up to 200 pounds
• Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
