About The Position
This full-time Registered Nurse position is responsible for case management within the home health setting. The role involves developing and maintaining therapeutic relationships with patients and families, providing individualized care, and mobilizing appropriate resources to ensure quality, cost-effective care. The nurse will adapt practice to the latest standards, participate in quality improvement initiatives, and educate patients, families, and colleagues. Key responsibilities include utilizing the nursing process for problem-solving, making sound clinical decisions for complex patient issues, prioritizing care, initiating referrals, and coordinating care through an interdisciplinary approach. The position also requires leadership behaviors, collaboration with healthcare team members, and a commitment to professional development, including achieving required certifications or a Bachelor's Degree and earning continuing education credits.
Requirements
- Nurse, Registered (RN) Licensure
- Graduate from an accredited school of nursing
- Earn 10 continuing education credits annually
- One year of medical surgical experience
- Has as demonstrated proficiency for the requirements for RN role
- Good communication and interpersonal skills
- Good documentation skills.
- Computer skills
- Good time management and organizational skills
- Familiarity with patient care equipment
- Current Illinois RN license
- Current CPR
