Requirements

  • Valid Oregon RN License.
  • Current Driver's License and reliable mode of transport.
  • Valid Auto Insurance.
  • Up-to-date CPR certification.
  • Proven experience as a Registered Nurse in a home health, acute care, or community health setting.
  • Excellent organizational and time-management skills.
  • Strong interpersonal and communication skills.
  • Ability to make decisions and work independently.
  • Physical endurance, capable of frequent travel and navigating various home environments.
  • Knowledge of best practices in home healthcare and patient-centered care.
  • Empathy and a deep commitment to patient welfare and sensitivity to their needs.

Responsibilities

  • Evaluate and assess the patient's health condition during each home visit.
  • Create and update comprehensive, individualized care plans in collaboration with physicians and other healthcare professionals.
  • Ensure adherence to all safety protocols and maintain high standards of hygiene and cleanliness during home visits.
  • Administer medication as prescribed and monitor responses to treatment.
  • Educate patients and their families on disease prevention and health promotion.
  • Provide emotional and psychological support to patients and their families.
  • Maintain detailed records of patient's health progress and treatment plans.
  • Coordinate with healthcare providers to ensure patient care is aligned with overall health goals.
  • Manage a caseload of patients, prioritizing needs based on severity and urgency.
  • Respond to emergency health issues as they arise and take decisive steps to address them.
  • Advocate for patients' needs and assist in navigating the complexities of home healthcare.
  • Conduct regular follow-up visits to assess progress and adjust care plans as needed.
  • Seeing patients in their home, documenting visits accordingly, and coordinating visits with providers.
