Registered Nurse - Home Health Case Manager
About The Position
Saint Luke’s Home Health is seeking a Full-Time Day RN to deliver exceptional care in patients' homes. Saint Luke’s Home Care & Hospice serves multiple counties in Kansas and Missouri. Our specially trained nurses, therapists, and other skilled staff have delivered high-quality care to patients in the comfort of their own homes and under their physicians' direct supervision for more than 30 years. And as part of Saint Luke’s Health System, we deliver the same high-quality, comprehensive, and compassionate care. A Home Health Registered Nurse functions independently and is responsible for providing patient care in the home setting.
Requirements
- RN MO and KS or Compact required
- BLS required
- Driver's License required
- 1 year of applicable experience
- Basic Life Support - American Heart Association or Red Cross
- Driver's License - Various
- Registered Nurse - Various
Responsibilities
- Utilizing the nursing process throughout the provision of patient/family care.
- Coordinating the care of patients and consulting other disciplines of the health care team.
- Facilitating patient care, anticipating patient care needs, individualizing patient care and adjusting or modifying the nursing plan of care as appropriate.
- Collaborating with the medical staff and the other disciplines to provide and enhance patient care.
- Adhering to all nursing policies and standards and proactively strives to maintain and improve quality outcomes.
