About The Position

This full-time Registered Nurse position focuses on Home Health Case Management within the Southwest Suburbs, covering territories including New Lenox, Homer Glen, Lockport, Joliet, and Crest Hill. The role involves developing and maintaining therapeutic relationships with patients and families throughout their healthcare journey, providing individualized care, and mobilizing resources to ensure quality, cost-effective care. The position requires adapting to the latest evidence-based standards, participating in quality improvement initiatives, and educating patients, families, and colleagues. The nurse will utilize the nursing process for problem-solving, making sound clinical decisions for complex patient cases, and coordinating care through an interdisciplinary approach. Leadership, collaboration, and professional development are key aspects, including participation in unit meetings, maintaining current knowledge, and contributing to a collaborative healthcare team environment. A sign-on bonus of up to $10,000 is available for eligible candidates.

Requirements

Nurse, Registered (RN)
A valid driver’s license issued by the Division of Motor Vehicles.
Graduate from an accredited school of nursing
Earn 10 continuing education credits annually
One year of medical surgical experience
Has as demonstrated proficiency for the requirements for RN role
Good communication and interpersonal skills
Good documentation skills.
Computer skills
Good time management and organizational skills
Familiarity with patient care equipment
Current Illinois RN license
Current CPR
