About The Position

This full-time Registered Nurse position focuses on Home Health Case Management, covering the Southwest Suburbs including New Lenox, Homer Glen, Lockport, Joliet, and Crest Hill. The role involves developing and maintaining therapeutic relationships with patients and families, providing individualized care, and mobilizing resources to ensure quality, cost-effective care. The nurse will also be involved in patient and staff education, utilizing the nursing process for problem-solving, and demonstrating leadership and collaboration skills. The position requires a commitment to professional development and adherence to ethical decision-making.

Requirements

Graduate from an accredited school of nursing
One year of medical surgical experience
Current Illinois RN license
Current CPR
A valid driver’s license issued by the Division of Motor Vehicles.
Good communication and interpersonal skills
Good documentation skills.
Computer skills
Good time management and organizational skills
Familiarity with patient care equipment

Nice To Haves

Earns a minimum of 10 contact hours per year
Has as demonstrated proficiency for the requirements for RN role
