Humber River Health is seeking a Registered Nurse for their Home Hemodialysis Unit within the Nephrology Program. This role involves addressing the needs of a unique patient population using a case-managed approach. The nurse will coordinate clinical activities, serve as a clinical resource, and work directly with patients and the team to ensure timely and appropriate treatment and follow-up. Responsibilities include autonomously assessing and educating patients, family members, and Personal Support Workers (PSWs) on home therapies, ensuring they have the necessary knowledge and skills for independent dialysis. The nurse will also participate in rotational on-call support for the program and liaise closely with the multidisciplinary team. The role encompasses a comprehensive approach to care for patients with chronic kidney disease who wish to dialyze independently or with assistance at home. This includes assessing referred patients, visiting and assessing home environments, providing training, and ongoing primary case management. As part of the Nephrology Program, the nurse will utilize a chronic disease prevention and management (CDPM) and self-care philosophy, contributing to the Ontario Renal Network (ORN) strategic plan. This involves championing initiatives like Person Centered Decision Making (PCD) and Access to Kidney Transplantation (AKT).
