About The Position

Care Advantage Inc. is seeking a dynamic Registered Nurse (Home Care Supervisor) to join their Personal Care Team in South Hill VA and surrounding areas. This full-time opportunity is for an experienced nurse with leadership skills who wants to make a difference. The role involves being a clinical leader, mentor, and advocate for patients, caregivers, and the community. Responsibilities include patient care, family support, team leadership, caregiver guidance, performance oversight, in-home supervision, orientation and training, ensuring regulatory compliance, documentation, quality improvement, building community partnerships, and supporting organizational growth.

Requirements

  • Active RN license in Virginia (compact/multi-state accepted)
  • Minimum of 1 year in a nursing role (LPN-to-RN pathway acceptable)
  • Associate's Degree in Nursing or higher
  • Strong communication and presentation skills
  • Ability to work collaboratively across teams
  • Reliable transportation for home visits
  • Comfortable with technology (computer/iPad documentation)
  • Willingness to travel regularly within assigned area

Responsibilities

  • Meet new patients, explain services, and complete start-of-care assessments.
  • Develop individualized care plans addressing medical needs, personal preferences, and family involvement.
  • Track patient progress, adjust care plans, and report changes in condition.
  • Teach or supervise household members assisting with patient care.
  • Instruct Home Care Nurses, Aides, and Personal Care staff on care plan details and expectations.
  • Review caregiver documentation, monitor reliability, and provide constructive feedback.
  • Conduct supervisory visits while staff are delivering services.
  • Lead or oversee training sessions for new staff.
  • Ensure adherence to OSHA, DMAS, CCC Plus Program, and state licensure regulations.
  • Prepare and coordinate clinical notes, progress reports, and compliance records.
  • Participate in interdisciplinary team meetings and quality reviews.
  • Establish and maintain relationships with referral sources, community partners, and healthcare providers.
  • Represent Care Advantage in the community, promoting the company's mission.
  • Assist with organizational growth during mergers and acquisitions.
