Requirements

Active RN license in Virginia (compact/multi-state accepted)
Minimum of 1 year in a nursing role (LPN-to-RN pathway acceptable)
Associate's Degree in Nursing or higher
Strong communication and presentation skills
Ability to work collaboratively across teams
Reliable transportation for home visits
Comfortable with technology (computer/iPad documentation)
Willingness to travel regularly within assigned area

Responsibilities

Meet new patients, explain services, and complete start-of-care assessments.
Develop individualized care plans addressing medical needs, personal preferences, and family involvement.
Track patient progress, adjust care plans, and report changes in condition.
Teach or supervise household members assisting with patient care.
Instruct Home Care Nurses, Aides, and Personal Care staff on care plan details and expectations.
Review caregiver documentation, monitor reliability, and provide feedback.
Conduct in-home supervisory visits while staff are delivering services.
Lead or oversee training sessions for new staff, including safety protocols and clinical standards.
Ensure adherence to OSHA, DMAS, CCC Plus Program, and state licensure regulations.
Prepare and coordinate clinical notes, progress reports, and compliance records.
Participate in interdisciplinary team meetings and quality reviews.
Establish and maintain relationships with referral sources, community partners, and healthcare providers.
Represent Care Advantage in the community, promoting the company's mission.
Assist with organizational growth during mergers and acquisitions.
