About The Position

Mass General Brigham Home Care is seeking a full-time Registered Nurse to join the “Lighthouse” team. This role focuses on providing skilled nursing care in patients' homes, allowing them to recover from illness or injury and manage chronic diseases within their own environment. The position offers autonomy and the opportunity to lead with skill and heart, utilizing clinical expertise and compassion. Nurses will serve as case managers, overseeing the full plan of care and collaborating with LPNs, therapists, and social workers. The role involves approximately 5-6 patient visits per day, Monday through Friday, with dedicated time for documentation, chart review, and coordination. Mass General Brigham Home Care provides non-acute, medically necessary skilled care in the home, including skilled nursing care (IV drug administration, injections, tube feeding, wound care, care plan education), physical therapy, occupational therapy, speech language pathology, medical social services, and medical supplies. The ideal candidate is experienced, emotionally intelligent, thrives in autonomous roles, and is dedicated to high-quality, compassionate care. Candidates should be confident in their clinical decision-making, organized in their time management, and comfortable navigating a dynamic home-based environment.

Requirements

  • Graduate of an approved School of Nursing
  • Current Massachusetts license as Registered Nurse
  • Minimum of one-year previous nursing experience in acute care setting
  • Travel required within MGB Healthcare at Home geographic area
  • Ability to work with various computer software
  • Strong interpersonal and customer service skills
  • Ability to work with a diverse population
  • Ability to communicate effectively in writing, verbally and electronically
  • Ability to work as a member of an interdisciplinary team of health care providers
  • Ability to work independently, be self-directed and adapt to unpredictable circumstances
  • Ability to organize and prioritize work, and adapt to changing situations

Nice To Haves

  • Prior home care experience preferred
  • Knowledge of hand-held patient care computer devices preferred

Responsibilities

  • Serve as case managers, overseeing the full plan of care.
  • Collaborate closely with LPNs, therapists, and social workers.
  • Complete approximately 5-6 patient visits per day.
  • Perform skilled nursing care, including IV drug administration, injections, tube feeding, wound care, and care plan education.
  • Document patient care and review charts.
  • Coordinate patient care with other healthcare providers.
  • Order supplies and contact physician offices for non-urgent matters (with case manager support).
