About The Position

CoxHealth is seeking a Registered Nurse to serve as a Home Care Liaison. This role involves receiving and reviewing referrals for Home Care services, assessing the level of care needed in the home, communicating the benefits of home care, and providing necessary patient information to establish services. The position is full-time with a day shift schedule, working from 8:00 AM to 5:00 PM.

Requirements

• Graduate of an accredited Nursing program.
• Current RN license active in the state of Missouri.
• Active Driver's License.
• Excellent communication skills in speaking with hospital personnel, doctors, patients, families, and other CoxHealth at Home employees effectively and courteously.
• Must be professional, as well as able to successfully convey all available services and their benefits in an empathetic and caring manner.
• Must possess ability to be mobile within the hospital setting at least 80% of the time.
• Must have reliable transportation.

Nice To Haves

• Minimum one (1) year home care experience.

Responsibilities
