About The Position

Compassionate and experienced Registered Nurse with expertise in hospice admissions, patient assessments, and care coordination across hospitals, long-term care facilities, assisted living communities, and home settings. Skilled in evaluating patients’ physical, emotional, psychosocial, and spiritual needs; educating patients and families about hospice services; developing individualized plans of care; and collaborating with physicians and interdisciplinary teams to ensure seamless transitions and high-quality end-of-life care. Strong communicator dedicated to patient-centered care, quality improvement, and collaborative teamwork.

Requirements

  • Graduate of an accredited nursing school.
  • Current Registered Nurse (RN) license in the state of Ohio with no board actions.
  • Current CPR certification.
  • Computer skills sufficient to accurately document services and patient care.
  • Valid driver’s license and acceptable driving history.
  • Ability to travel as needed in varying weather and road conditions.
  • Current automobile insurance meeting state minimum requirements.
  • Ability to successfully complete a background investigation and 10-panel drug screening.

Nice To Haves

  • Two years of acute care nursing experience preferred.
  • Certification in Hospice and Palliative Medicine (CHPN) preferred.

Responsibilities

  • Conduct comprehensive patient and family assessments addressing physical, emotional, psychosocial, and spiritual needs in hospitals, long-term care facilities, assisted living communities, and home settings.
  • Educate patients and families regarding hospice services, eligibility, and available resources while supporting informed decision-making and a smooth admissions process.
  • Collaborate with interdisciplinary team members to develop and initiate individualized plans of care that reflect each patient’s goals, preferences, and clinical needs.
  • Communicate clinical findings and patient status to hospice physicians to support eligibility determination and certification.
  • Provide ongoing patient assessments, symptom management, and care coordination for patients in contracted facilities while facilitating transitions between care settings when needed.
  • Partner with physicians, hospital personnel, facility staff, and other care teams to promote continuity of care and timely discharge planning consistent with hospice principles.
  • Assist across departments and support organizational operations and patient care initiatives as needed.
  • Participate in quality improvement activities designed to enhance patient outcomes, patient and family experiences, and service excellence.
  • Mentor and orient newly hired staff, promoting teamwork, professional development, and positive working relationships.
