About The Position

CVP seeks Registered Nurses to join our team in providing our nation's Veterans with geriatric nursing services at the VA Palo Alto Health Care System, Menlo Park campus, located at 795 Willow Street, Menlo Park, CA 94025. This position supports the Community Living Center with potential assignment to the main Palo Alto campus as operational needs dictate. Pay: $53.44-60/hr

Requirements

  • U.S. Citizenship required; ability to successfully undergo Government sponsored background investigation.
  • Associate Degree or Bachelor of Science in Nursing from a program accredited by CCNE, ACEN, or other U.S. Department of Education recognized accrediting body; Bachelor's degree preferred.
  • Current, full, active, and unrestricted RN license in any U.S. State, Territory, Commonwealth, or District of Columbia; must maintain throughout employment.
  • Minimum 2 years recent experience in geriatric nursing care.
  • Ability to speak, understand, read, and write English fluently.
  • Current Basic Life Support (BLS) certification from American Heart Association or American Red Cross required;
  • Current nursing license with no negative indicators; completion of standard infection control measures including current PPD, influenza vaccination (unless contraindicated), and blood-borne pathogen training.
  • Physical ability to sit, stand, walk, lift up to 50 lbs from floor to waist, carry up to 40 lbs, and push/pull with 30 lbs of force during work shift.

Nice To Haves

  • Advanced Cardiac Life Support (ACLS) certification preferred.

Responsibilities

  • Assess patient/resident health care needs, develop and maintain individualized care plans, and act on significant changes in patient/resident health conditions in accordance with VA policies and procedures.
  • Perform comprehensive head-to-toe assessments, administer medications (including IV), insert and manage IV lines, peripheral and central lines (including PICC lines), and coordinate patient admissions and discharges.
  • Conduct wound care including sterile and non-sterile dressing changes, tracheostomy care, ventilator care, suctioning (oral, nasogastric, and tracheostomy), tube feeding administration, and specimen collection.
  • Monitor vital signs/hemodynamic status, administer blood products, and respond to emergency situations while providing clinical oversight to LPNs, Nursing Assistants, and Health Technicians.
  • Utilize Bar Code Medication Administration (BCMA) with 100% accuracy, perform Braden Scale assessments, Fall Risk Assessments, skin assessments, and clear all CPRS clinical alerts daily.
  • Document patient care activities accurately and timely in the electronic medical record, educate patients/residents and families regarding health conditions and medications, and maintain privacy and confidentiality standards.
  • Practice VA core values (ICARE), report abuse incidents immediately, complete mandatory VA training requirements, and maintain PPE and hand hygiene compliance.
