We have a new opening for a Registered Nurse - Discharge Planner at Nellis AFB in North Las Vegas, NV. This role provides discharge planning services to patients admitted to the medical treatment facility, including inpatients, same-day surgery, and emergency department patients. The position involves triaging discharge planning needs, developing efficient processes, and accurately predicting discharge/transfer requirements to ensure safe and timely transitions. The Discharge Planner facilitates smooth transitions by coordinating with patients, families, caregivers, and the multidisciplinary healthcare team, addressing complex medical and psychosocial needs. Collaboration with utilization management, case management, and social work is essential for appropriate and safe discharges in accordance with DoD/W regulations. The role also involves applying evidence-based programs to monitor Length of Stay (LOS) and admission/discharge criteria, arranging transfers, surveying the continuum of care for quality concerns, and participating in various committee meetings. Responsibilities include coordinating with clinical units, monitoring LOS trends, liaising with third-party payers and TRICARE, validating insurance benefits, and providing ongoing education to patients, providers, and staff on discharge planning and post-discharge care. The role also includes teaching medical students, residents, nurses, and other healthcare personnel about discharge planning processes and orienting new personnel. Establishing good interpersonal relationships, effective communication, accurate documentation, and contributing to a professional environment are key. The role may require working various shifts or augmenting clinical nursing support within the Medical Management Department and ensuring a safe work environment in accordance with regulatory agencies and policies.
