About The Position

The Registered Nurse Coordinator (RN) for the Transfer Center is responsible for coordinating all aspects of Transfer Center transfers, from the initial call through patient discharge and follow-up. This role ensures the highest quality of service is provided to physicians, nurses, patients, and other medical professionals involved in the transfer process.

Requirements

• Associates Degree in Nursing, or equivalent, from an accredited program.
• 4 years of clinical experience in an acute care setting.
• Current Arizona, or other compact state, Registered Nurse license.
• BLS certification.

Nice To Haves

• Bachelor of Science in Nursing.
• RN experience in an ER/Trauma, Neurosciences, and/or Cardiovascular setting.
• Knowledge of, and the ability to apply, InterQual criteria.
• ACLS certification.
• Applicable professional certification (e.g., CCRN, CEN, CNRN).
• Ability to interact and communicate in English, both orally and written, with all levels of staff, physicians, patients, and families in an effective manner, exhibiting tact, enthusiasm, and patience.
• Sensitivity to cultural and bilingual issues.

Responsibilities

• Respond to all referrals to the Transfer Center program in a timely manner.
• Contact the receiving nurse to outline the patient’s condition, medical treatment plan, and physical orders to prepare him/her for the patient’s arrival.
• Assist with the development and implementation of educational programs for nurses (e.g., Lunch & Learns, ACNS, etc.).
• Facilitate physician-to-physician communication.
• Communicate with the sending facility to assure follow-up with the Physician is accomplished.
• Assist the sending facility with the use of air or ground transport of the patient.
• Contact the Admitting Department to admit the patient.
• Perform other related job tasks or responsibilities as assigned.
