About The Position

The Registered Nurse Clinician - Home IV provides medically prescribed infusion therapy to patients with acute and chronic conditions in the home or ambulatory setting. This role involves working collaboratively as a member of an interdisciplinary team, utilizing the nursing process to assess and care for patients, and providing comprehensive nursing care in a professional, ethical, knowledgeable, and compassionate manner. The nurse will also provide disease and drug-specific education to patients and caregivers, comply with HIPAA and ACHC regulations, and travel to patient homes and outlying clinics as needed. Maintaining knowledge of medical/nursing practices through educational in-services is essential. The position requires working on an as-needed basis according to patient volume in home infusion, with the Director coordinating the schedule and needs. Upholding the Palmetto Mission, Vision, and Values while providing safe and competent care is paramount. Specific nursing duties include accessing and maintaining ports, peripheral lines, PICC lines, and Midlines according to INS Standards; obtaining lab specimens; collecting vital signs and ongoing patient monitoring; safely administering intravenous infusion and injectable medications; documenting patient care accurately and legibly; responding to and reporting adverse drug reactions; assisting with administrative duties; documenting according to DHEC and ACHS compliance measures; following Bag Technique and standard precautions; following Hazardous Drug spill procedures; and notifying the MD and assisting in coordination of care in the event of an emergency or urgent matter.

Requirements

  • Graduate from an accredited school of nursing.
  • Active Registered Nurse license in the state(s) of practice.
  • 1 year of nursing experience in a clinical specialty involving infusion therapy.
  • BLS or obtain upon hire.
  • Proficient at partner programs (i.e.: MS Outlook, Word, Excel).
  • Ability to cope with ambiguous, changing environment while under pressure in a calm manner.
  • Must be able to work as a team member and develop productive and cooperative working relationships with all members of the Palmetto Infusion team.

Responsibilities

  • Provide medically prescribed infusion therapy to patients with acute and chronic conditions in the home or ambulatory setting.
  • Work collaboratively as a member of an interdisciplinary team in a home setting or ambulatory infusion clinic.
  • Effectively utilize the nursing process to assess and care for patients.
  • Provide comprehensive nursing care in a professional, ethical, knowledgeable, and compassionate manner.
  • Provide disease and drug specific education to patients and caregivers.
  • Comply with HIPAA and ACHC regulations.
  • Travel to patient homes and deliver care according to plan of care and providers orders.
  • Travel to outlying clinics as needed.
  • Maintain knowledge of medical/nursing practices by participating in educational in-services.
  • Work on an as needed basis according to patient volume in home infusion.
  • Provide safe and competent care while in the home or ambulatory setting at all times upholding the Palmetto Mission, Vision, and Values.
  • Access and maintain ports, peripheral lines, PICC lines, Midline, Broviac according to INS Standards.
  • Obtain lab specimens.
  • Collect vital signs and ongoing monitoring of the patient.
  • Safely administer intravenous infusion and injectable medications as ordered by prescribing physicians.
  • Document patient care appropriately and legibly.
  • Respond to and accurately report adverse drug reactions to the Director of Home Infusion.
  • Assist with administrative duties as needed, such as scan charts, call patients, review labs, etc.
  • Document according to DHEC and ACHS compliance measures.
  • Follow Bag Technique and standard precautions while in the home setting.
  • Follow Hazardous Drug spill according to provided training annually and OSHA regulations.
  • Notify MD and assist in coordination of care in the event of emergency or urgent matter while in the home.
