About The Position

This role is for a Registered Nurse in the Primary Care specialty at Saint Thomas Medical Partners Nashville. The nurse will be responsible for bringing patient care plans to life by implementing, monitoring, and evolving them to meet unique patient needs. They will act as a vigilant communicator, monitoring conditions and relaying vital updates to the team. The role also involves interpreting and carrying out physician and nursing orders with clinical accuracy, and leading the discharge planning process from day one to ensure a safe transition home for patients.

Requirements

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date (American Heart Association or American Red Cross accepted).
• Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date.
• Diploma from an accredited school/college of nursing required OR Required professional licensure at time of hire.

Responsibilities

• Implement, monitor, and evolve patient care plans.
• Monitor patient conditions and communicate vital updates to the team.
• Interpret and carry out physician and nursing orders with clinical accuracy.
• Lead the discharge planning process, collaborating with the care team for a safe transition home.
