About The Position

This position offers an opportunity to work closely with our Family Medicine and Pediatric providers in an outpatient setting, ensuring seamless coordination of care throughout the system. The role involves managing patient workflows, assisting with scheduling, triage phone calls, working independently off of standing orders, Transitional Care Management calls, minor procedure assisting and delegation of tasks to assistive personnel. Requirements

  • Graduated from an accredited school of Nursing.
  • Registered Nurse (RN) license required.
  • 1 year of prior experience in nursing.
  • Current BLS provider status in accordance with American Heart Association (AHA) guidelines.
  • Strong organizational and communication skills.
  • Strong attention to detail is essential to ensure quality and safe care.
  • Ability to multi-task and work in a fast-paced clinical environment.

Nice To Haves

  • Experience in ambulatory setting preferred.

Responsibilities

  • Provide professional nursing care to our family medicine and pediatric patients in a clinical setting.
  • Coordinate complex referral and scheduling needs for patients.
  • Manage task lists for physicians.
  • Handle results, coordinating follow-up care and referrals.
  • Oversee chronic disease registry needs.
  • Assist with outreach clinic/hospital services and patient follow-ups.
  • Collaborate with interdisciplinary teams to optimize patient care.
  • Managing patient workflows.
