About The Position

The Program of All-Inclusive Care for the Elderly (PACE) offers a variety of services, with many of them provided onsite at a PACE Center. PACE is an alternative to nursing home care and, through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides, coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

Requirements

Associate degree required.
RN–Nursing Diploma or Degree from an accredited School of Nursing or University.
Proficiency with Microsoft Word and Outlook.
Well-developed ability to show discretion and maintain confidentiality.
Strong communication skills, both written and verbal.
Minimum of one year of experience working with the frail, elderly, or long-term care population.
Current AHA CPR certification, or willingness and ability to obtain certification through Senior Care Partners, PACE.
Ability to practice within the appropriate scope of authority.
Ability to successfully complete position-specific competencies established by Senior Care Partners, PACE and approved by CMS before working independently.
Knowledge of current medical practices, procedures, and applicable long-term care laws, regulations, and guidelines.
Ability to make sound independent clinical decisions when circumstances require.
Strong interpersonal skills and the ability to communicate respectfully with participants, families, staff, government representatives, visitors, and community partners.
Ability to plan, organize, implement, and support organizational goals, policies, procedures, and quality-care initiatives.
Patience, tact, enthusiasm, and a positive disposition when working with participants and families in challenging circumstances.

Must have reliable transportation, current valid Michigan driver’s license and car insurance.
Must be 18 years of age or older.
Must be fully vaccinated, or willing to be fully vaccinated against the following communicable diseases: Influenza, MMR, Varicella, Tdap/Td, Covid-19 and Hepatitis B.

Nice To Haves

Bachelor’s of Science in Nursing preferred.
Minimum of one year home care experience preferred.
Minimum of one-year wound care experience preferred.

Responsibilities

Provides direct nursing care to participants.
Works independently in the application of advanced nursing knowledge and skills and can manage complex clinical situations.
Collaborates with both licensed and non-licensed personnel.
Triages emergent situations and is able to remain independent in the application of advanced nursing knowledge.
Monitors acute and chronic medical conditions in coordination with Primary Care Providers (PCPs).
Utilizes care plans independently to provide clinic and home-based direct care.
Administers treatments, procedures, and medications as prescribed to participants.
Performs routine monitoring of acute and chronic medical conditions following the participants' goals of care.
Performs wound care and treatments as prescribed in the participant's plan of care.
Administers medications, immunizations, and other care as prescribed.
Performs assessments and actively participates in care plans.
Makes home visits regularly to provide care and treatment as ordered by the PCP. This may include wound care, lab draws, nurse assessments, and other nursing tasks.
Performs on-call needs and determines immediate needs for the participant as needed.
Works beyond normal working hours and on weekends and holidays, as necessary.
Must complete all required education and employee file requirements on time.
Documents in the participant’s medical record as required by policy.
Cares for, supports, and collaborates with everyone involved in the participant’s care, including staff and the participant’s family, using the participant-centered care model.
Keeps the leader informed of problems or issues.
Performs other duties as assigned.
