About The Position

Mass General Brigham Home Care is seeking a full-time Registered Nurse to join the “Logan” team. This role focuses on providing skilled nursing care in patients' homes, transforming care in the community. The position involves serving as a case manager, overseeing the full plan of care, and collaborating with LPNs, therapists, and social workers. Nurses typically complete 5-6 visits per day, Monday through Friday, with dedicated time for documentation, chart review, and coordination. The role requires travel within the MGB Healthcare at Home geographic area, primarily serving East Boston and Winthrop, with support for Chelsea and Revere. The organization provides resources such as an IV specialist, wound care specialist, and case manager support, along with educational assistance from the wound and IV team. Case managers assist with ordering supplies and contacting physician offices for non-urgent matters. Mass General Brigham Home Care provides non-acute, medically necessary skilled care in the home to help “homebound” patients recover from illness or injury and manage exacerbations of chronic disease. Services include skilled nursing care (IV drug administration, injections, tube feeding, wound care, care plan education), physical therapy, occupational therapy, speech language pathology, medical social services, and as needed medical supplies. Requirements

  • Graduate of an approved School of Nursing
  • Current Massachusetts license as Registered Nurse
  • Minimum of one-year previous nursing experience in acute care setting.
  • Travel required within MGB Healthcare at Home geographic area
  • Ability to work with various computer software
  • Strong interpersonal and customer service skills
  • Ability to work with a diverse population
  • Ability to communicate effectively in writing, verbally and electronically
  • Ability to work as a member of an interdisciplinary team of health care providers
  • Ability to work independently, be self-directed and adapt to unpredictable circumstances
  • Ability to organize and prioritize work, and adapt to changing situations

Nice To Haves

  • Prior home care experience preferred
  • Knowledge of hand-held patient care computer devices preferred
