About The Position

Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge. Architect comprehensive care plans that bridge the gap between hospital and home, ensuring every step aligns with the patient’s personal values and recovery goals. Advocate fiercely for your patients by identifying and removing systemic barriers to care, ensuring they have the essential resources needed for a safe transition. Orchestrate clinical communication across the multidisciplinary team to ensure the health plan is executed seamlessly and no detail is overlooked. Champion documentation integrity by serving as a clinical resource, ensuring medical records accurately reflect the necessity of services and the complexity of the patient's journey.

Requirements

  • Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required.
  • Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Nice To Haves

  • BLS Provider preferred. American Heart Association or American Red Cross accepted.
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
