About The Position

Optimal Hospice is seeking compassionate Registered Nurses (RNs) to join their team as Case Managers for the North Denver Metro area. This role is focused on providing hospice care to patients, helping them achieve and maintain the best possible quality of life. Requirements

  • Registered nurse with current licensure to practice professional nursing in the state.
  • Graduate of National League for Nursing accredited school of nursing.
  • Maintains a current CPR certification.
  • Current valid driver’s license.
  • Demonstrates excellent observation, problem solving, verbal and written communications, nursing skills per competency checklist.
  • Shows ability to organize and prioritize workload independently.

  • At least one year of Hospice experience

Responsibilities

  • Develops and maintains a therapeutic relationship through the health care continuum.
  • Provide direct patient care services to hospice patients.
  • Empower patient's caregivers by educating them on care for their loved one.
  • Provide coordination of care, ensuring continuous assessment of each patient's and family's needs.
  • Responsible for documentation and communication of care, medication, and observations made during patient care to track responsiveness and health.
  • Responsible for providing appropriate patient/family education to help patient/family achieve a pain-free and comfortable status.
  • Manage pain and symptoms so patients can make the most of every day, focusing on living well and with dignity.
  • Provide compassionate care to patients and their family members at end of life.
  • Document and connect with the care team through point-of-care documentation.
