The Hospice Registered Nurse Case Manager will conduct comprehensive patient assessments, including vital signs and medical history review. This role involves traveling to patients' residences to conduct visits and develop individualized care plans in collaboration with multidisciplinary teams. The RNCM will coordinate hospice care by working with other team members and the patient's family members to ensure needs are met. Responsibilities include performing evaluations for hospice care and informational visits, consulting with physicians regarding changes in patient condition, and educating patients and families on treatment plans, medication management, and health maintenance strategies. The role also requires submitting complete, accurate, and relevant clinical notes promptly, participating in IDG and case conferences, and performing on-call duties as needed. The RNCM will develop and implement the initial plan of care and supervise and evaluate the care given by the Hospice Aide.
