About The Position

Boost Home Healthcare is hiring an experienced Registered Nurse Case Manager to serve clients in Fort Collins, Colorado, and surrounding areas. The role offers the independence to manage a client caseload, guide caregivers, and build relationships with clients and families, with support from the organization. The position is full-time, Monday through Friday, with day shifts from 8 a.m. to 5 p.m. Scheduling is flexible, with occasional holidays and a weekend rotation. Pay is $47–$52 per hour, based on experience.

Requirements

Active, unencumbered Registered Nurse (RN) license in Colorado.
At least one year of home care or home health nursing experience.
Case management and care coordination experience.
Current BLS/CPR certification.
Valid driver’s license and reliable transportation.
Strong clinical judgment, organization, and communication skills.
Genuine passion for client-centered care.

Responsibilities

Complete initial and ongoing in-home nursing assessments of clients’ physical, cognitive, psychosocial, and home safety needs.
Create and update individualized plans of care, and ensure caregivers understand and follow them.
Manage a caseload of home care clients, track changes in condition, and adjust services as needs change.
Serve as the main clinical contact for families, physicians, hospitals, and community partners; make referrals when needed.
Maintain accurate, timely client records and follow Colorado home care regulations, infection control requirements, and HIPAA guidelines.
Help resolve client and caregiver concerns promptly and professionally through on-call support.
