About The Position

Optimal Hospice is seeking a compassionate Registered Nurse Case Manager to join their growing team in the North Denver Metro area. This role focuses on providing direct patient care services to hospice patients, empowering caregivers through education, and coordinating care to ensure continuous assessment of patient and family needs. The position involves documenting care, medications, and observations to track patient responsiveness and health, and providing education to help patients achieve a pain-free and comfortable status. The Registered Hospice Nurse will manage pain and symptoms so patients can make the most of every day, focusing on living well and with dignity, and will document and connect with the care team through point-of-care documentation.

Requirements

  • Registered nurse with current licensure to practice professional nursing in the state.
  • Graduate of National League for Nursing accredited school of nursing.
  • Maintains a current CPR certification.
  • Current valid driver’s license.
  • Demonstrates excellent observation, problem solving, verbal and written communications, nursing skills per competency checklist.
  • Shows ability to organize and prioritize workload independently.

Responsibilities

  • Develops and maintains a therapeutic relationship through the health care continuum
  • Provide direct patient care services to hospice patients
  • Empower patient's caregivers by education them on care for their loved one
  • Provide coordination of care, ensure continuous assessment of each patient's and family's needs
  • Responsible for documentation and communication of care, medication, and observations made during care of the patient in order to track responsiveness and health of patient.
  • Responsible for providing appropriate patient/family education to help patient/family achieve a pain-free and comfortable status.
  • Manage pain and symptoms so patients can make the most of every day, focusing on living well and with dignity.
  • Provide compassionate care to patients and their family members at end of life.
  • Document and connect with the care team thru point of care documentation.
