Registered Nurse (Per Diem)
About The Position
Hospice of the West is seeking a per diem Hospice RN Case Manager (RNCM) to join their dedicated nursing team. This role provides end-of-life care throughout the community, focusing on supporting patients and families through meaningful moments of care. The RNCM will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through their hospice journey as part of an interdisciplinary care team. Requirements
- Current unencumbered license as a Registered Nurse.
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
- Must be computer proficient in typing and various programs, including background in EMR.
- Possess and maintains current CPR certification if required by state.
- Current driver's license, vehicle insurance, and access to dependable transportation.
Nice To Haves
- Hospice experience is a plus.
Responsibilities
- Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
- Partner closely with the Clinical Director to assess the eligibility and suitability of clients for specialized hospice services.
- Deliver hands-on hospice nursing care, coordinate services, and guide patients and families through their hospice journey.
- Apply nursing skills by delivering top-notch care rooted in proven principles and techniques to ease pain and bring solace.
- Empower patients and families through education, encouraging their active participation in creating personalized care plans.
- Share the responsibility of call duties.
