About The Position
We are seeking an experienced Registered Nurse Case Manager to join a fast-paced acute care environment. The ideal candidate will have a strong clinical background combined with expertise in care coordination, utilization review, and discharge planning.
Requirements
- Active California RN license (required at time of submission)
- Minimum required experience in acute care settings, including Medical-Surgical, Telemetry, ICU, and some ER exposure
- Prior case management experience within California (required)
- Strong background in care coordination, utilization review, and discharge planning (required)
- Proficiency with InterQual and Epic systems (required)
- Previous travel experience required (first-time travelers not accepted)
Responsibilities
- Coordinate patient care across the continuum to ensure optimal outcomes
- Perform utilization review and apply clinical guidelines (e.g., InterQual)
- Develop and implement discharge plans in collaboration with interdisciplinary teams
- Communicate effectively with patients, families, and healthcare providers
- Maintain accurate documentation using electronic medical records
- Ensure compliance with hospital policies and regulatory standards
