We’re seeking a compassionate and experienced RN Case Manager to join our remote care team. In this role, you’ll collaborate with members telephonically and occasionally face‑to‑face to assess needs, coordinate care, and promote overall health and wellness. This is a fully remote position—candidates may live in any state and must be available to work Monday–Friday, 9:00 AM–5:30 PM EST for a scheduled 8‑hour shift. Weekends and holidays may be required based on the needs of the department. Currently, both weekends and holidays are on-call only and covered on a volunteer basis.

Requirements

  • Active, unrestricted RN license in your state of residence
  • Willingness to obtain additional state licenses (company‑paid)
  • 3+ years of acute care RN experience (medical, surgical, ICU, pediatrics, case management, or discharge planning)
  • Comfortable working with multiple systems/screens simultaneously
  • Private, distraction‑free home workspace with high‑speed internet

Nice To Haves

  • Compact RN license
  • 1+ year of case management experience
  • Certified Case Manager (CCM)
  • Strong customer service and telephonic communication skills
  • Proficiency with Microsoft Office (Word, Excel, PowerPoint)

Responsibilities

  • Assess, plan, and coordinate care to support member health and wellness
  • Develop proactive care plans to improve short‑ and long‑term outcomes
  • Review clinical data (assessments, claims, etc.) and connect members to available programs and resources
  • Apply clinical judgment to address complex medical and social needs
  • Engage members using strong interviewing and communication skills
  • Ensure compliance with regulatory and company case management standards
