About The Position
Baptist Health is seeking a Registered Nurse Case Manager for the In-Patient Case Management Department at Baptist Medical Center Jacksonville’s Downtown campus. This is a full-time, day-shift position working Monday through Friday. The role coordinates care for hospitalized patients, conducts high-risk screenings and assessments, arranges post-acute care services with appropriate documentation, and collaborates with multidisciplinary team members.
Requirements
- One year of case management experience.
- Associate’s Degree in Nursing (ADN).
- Basic Life Support (BLS).
- Driver’s license (DMV).
- Active Florida Registered Nurse (RN) license.
Responsibilities
- Coordinate care for hospitalized patients.
- Conduct high-risk screenings and assessments.
- Coordinate post-acute care services and provide proper documentation.
- Collaborate with multidisciplinary team members.
