About The Position

Optimal Hospice is experiencing incredible growth and is inviting dedicated Registered Nurses to join their team. This role involves providing support with visits in the CO Springs area. Optimal Hospice is devoted to helping patients achieve and maintain the best possible quality of life through quality patient care, innovative specialty health care programs, and state-of-the-art technology. Requirements

• Registered nurse with current licensure to practice professional nursing in the state.
• Graduate of National League for Nursing accredited school of nursing.
• Maintains a current CPR certification.
• Current valid driver’s license.
• Demonstrates excellent observation, problem solving, verbal and written communications, nursing skills per competency checklist.
• Shows ability to organize and prioritize workload independently.

Nice To Haves

• At least one year of Hospice experience

Responsibilities

• Develops and maintains a therapeutic relationship through the health care continuum.
• Provide direct patient care services to hospice patients.
• Empower patient's caregivers by educating them on care for their loved one.
• Provide coordination of care, ensure continuous assessment of each patient's and family's needs.
• Responsible for documentation and communication of care, medication, and observations made during care of the patient in order to track responsiveness and health of patient.
• Responsible for providing appropriate patient/family education to help patient/family achieve a pain-free and comfortable status.
• Manage pain and symptoms so patients can make the most of every day, focusing on living well and with dignity.
• Provide compassionate care to patients and their family members at end of life.
• Document and connect with the care team through point of care documentation.
