About The Position

At St. Croix Hospice, the Registered Nurse Case Manager provides individualized, holistic support to patients in their homes. The role collaborates with the care team to develop personalized plans of care, leads end-of-life conversations, and supports effective symptom management with empathy and clinical excellence. The position serves patients and families through the end-of-life journey, helping promote dignity, comfort, and respect.

Requirements

• Associate, bachelor's, or master's degree from an accredited nursing program and registered nurse (RN) qualification.
• At least 1 year of recent acute care experience.
• Reliable transportation, a valid driver's license, and proof of auto insurance.

Nice To Haves

• Hospice or home health experience.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patients and families and update them as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
