About The Position

Right at Home Boston Northwest is seeking a compassionate and organized Registered Nurse (RN) / Case Manager to join their Home Care team. This role involves coordinating high-quality care, supporting caregivers, and ensuring clients remain safe and comfortable at home. The position offers a schedule of 24-40 hours per week and is based in Bedford, MA, and surrounding communities.

Requirements

• Active, unrestricted Registered Nurse (RN) license in Massachusetts.
• Two or more years of nursing experience.
• Valid driver’s license, insured vehicle, and ability to travel locally to clients’ homes.

Responsibilities

• Conduct in-home assessments and reassessments and develop or update individualized care plans.
• Provide ongoing clinical case management, follow-up visits, and timely documentation.
• Support and educate caregivers, respond to changes in condition, and help ensure care plans are followed.
• Serve as a trusted clinical liaison for clients, families, caregivers, physicians, and other healthcare professionals.
• Collaborate with the Director of Nursing, scheduling, and care management teams to support continuity of care.
• Help address client and family concerns and coordinate appropriate clinical follow-up.
• Partner with community referral sources and support smooth care transitions for new and existing clients.
