About The Position

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide individualized, holistic support to patients in their homes. The role collaborates with a care team to develop and update personalized plans of care, leads end-of-life conversations, and supports effective symptom management with empathy and clinical excellence. The position is full time, based in Atchison, Kansas, and includes on-call expectations and 100% travel.

Requirements

• Associate, bachelor's, or master's degree from an accredited nursing program (RN).
• At least 1 year of recent acute care experience.
• Reliable transportation, a valid driver's license, and proof of auto insurance.

Nice To Haves

• Hospice or home health experience.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patients and families, updating them as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
