About The Position

Dove Healthcare – West Eau Claire is seeking a Registered Nurse (RN) Case Manager to join our team. The position involves coordinating and overseeing care to ensure quality outcomes for residents, supporting palliative care services focused on comfort, symptom management, and quality of life, and collaborating with rehabilitation teams to help residents reach their highest level of function. The RN Case Manager will supervise day-to-day care and team member performance, ensure care plans are followed, updated, and compliant with regulations, and assist with staffing coordination and team support. A key aspect of the role is fostering a safe, comfortable, and home-like environment for residents and participating in an on-call rotation.

Requirements

Current Wisconsin RN license
Graduate of an accredited nursing program
Previous experience in a related setting
Strong leadership, organization, and problem-solving skills
Ability to manage priorities in a fast-paced environment
Team-oriented with excellent communication skills
Passion for providing high-quality, compassionate care
